Nurse Driven Community Care

We understand that often family, work and other life events dominate daily life. However, preventative wellness is essential for everyone despite your financial circumstances. This non for profit organization works with home health, community food pantries and community events to create safe spaces to address your healthcare needs. Lack of access, transportation, representation and insurance are all known as social determinants of health. We work to remove certain barriers so you can prioritize YOU. 

You are not just a number or what insurance is paying your visit. We strive to  provide health education, medical intervention, and resources to improve health outcomes.  Our providers are trained to address chronic conditions, physical wellness exams and holistic treatment.
